To help you become familiar with how we run our medical practice, here is our information about scheduling appointments, office hours, payments, and insurance along with our online forms.

Appointments
We are glad to make an appointment for you during our scheduled office hours. For serious illness and minor injuries, we do everything possible to see patients the same day. We would appreciate it if you could give us a call 24 hours in advance of not being able to make an appointment. That way, we have the chance to schedule another patient in that time slot.

Payments
Payment for services is due at the time of your visit and this includes co-payments, deductibles and services not covered by your insurance. For your convenience, we accept debit and credit cards as well as checks and cash. We also offer payment plans.
